Are you able to get Professional Medical help from a Doctor, or, perhaps you are able to go to a Sexual Health Center that you can travel to?

So, obviously, we can rule out anything sexually related if you have not had any sexual contact. This does look unusual but can have been caused by the way your penis has developed.
When did it first look like this, can you remember how long ago you first noticed this?
If you wanted to get this checked you would need to see a Doctor who may then refer you to a specialist. Is accessing a Doctor something you can easily arrange?

What you will need to be careful with is keeping this area clean and hygienic. You should wash the tip of your penis as often as you can. If you have medicated soap in the house you can use this to very carefully clean the tip area of your penis, but be sure to rinse the soap away with plenty of water. The best time to do this is when you take a shower use the soap, then plenty water to wash away any trace of the soap from the skin of your penis.

I agree with Wee Dugi , you should see an urologist. What you have may be caused by phimosis. Phimosis may be serious when left untreated. Your foreskin may have trapped some bacterias.
You should see an urologist so that he can help you expose your glans completely and have a better hygiene when you wash your penis.
