I can report a self experience with this issue that happened to me at age 23. Painless Blood in semen happened after vigorous masturbation. the bleeding lasted about 3 more weeks with masturbation until blood gradually turned black and stopped.
so the most likely reason was blood vessel rupture or urethral mucosal bleed post intense masturbation.
