Ayuda con problemas para tragar y dolores en el pecho..¿qué hago?

Tengo problemas para tragar desde hace años. Me ocurre cuando como o bebo. Puede pasar de repente. Siento como si me estuviera ahogando y luego tengo que vomitar inmediatamente para poder respirar. He aprendido que debo comer muy despacio y tomar un sorbo después de cada bocado. Lo he pasado por alto porque tenía mucho miedo de lo que podría ser. Pero ha empeorado mucho. Cuando como ciertos alimentos, puedo comer un bocado y empezar a ahogarme inmediatamente. También ya no puedo tomar pastillas de ningún tamaño. De nuevo, me ahogo inmediatamente. Lo que luego se convierte en vómitos y no poder tragar nada más por un tiempo. Esta noche estaba comiendo carne y sentí algo atascado en el pecho, no necesariamente hasta el punto de ahogarme todavía. Pero intenté pasarlo y luego me ahogué y vomité. Y nunca obtuve alivio. El dolor en el pecho es insoportable, sigo eructando bilis cada par de minutos. Todavía no puedo tragar nada sin ahogarme. Esto lleva 6 horas. Ahora son las 1:14 am y estoy sentado en el baño llorando. ¿Qué les parece esto? ¿Acalasia? ¿Alguna sugerencia sobre qué hacer para aliviar el dolor?

Yes, to me it does sound like Achalasia.  I have had it for over 25 years and that was how I started out.  When your food comes back up, it it soured with stomach acid, or has it just been stuck in your esophagus ?  If not soured, that would REALLY make me think it is Achalasia.  I was so afraid to get tested back then, because I worried it was a tumor or blockage.  But life is SO much better since I got treated.  I would encourage you to go see a gastroenterologist who has experience with Achalasia, and go thru the testing to find out.  It cannot be cured, but they can do a LOT to make things much much better.

Yo iría a un gastroenterólogo para obtener un diagnóstico. Podría ser que tengas una estenosis en el esófago que se pueda corregir. Investigaría a fondo la acalasia y sus tratamientos, así como las ventajas y desventajas de la dilatación antes del tratamiento quirúrgico. Saber con qué estás lidiando. No todos los gastroenterólogos son iguales y no muchos saben cómo tratar adecuadamente la acalasia.

There is obviously a severe reaction in your throat or oesophagus that needs urgent investigation by a doctor.   The doctor would examine you and consider what else it might be, because diagnosis of achalsia cannot really be confirmed until you have tests like a barium swallow, endoscopy and manometry.

​I think it would be a good idea to list the foods that give you this reaction, and any that do not.  

​Achalasia is caused by the nerve endings for the muscles in your oesophagus not making the progressive contractions to take food down into your stomach.   Often, the lower oesophageal sphincter is clamped tight shut and does not relax.   Building up food creates pressure that leads to regurgitation.   If pills or spicy food remain in your oesophagus, that can lead to inflammation, and this might be leading to some of the irritation / reaction.   When the muscles go into uncoordinated spasm it creates a pain in your chest that might feel as if you are having a heart attack.

​Meanwhile concentrate on relaxation and avoid tension (if possible).   Make sure you keep drinking, and concentrate on soft food, and take in nutrition with soup etc.   Do not keep swallowing if the food has not passed further into your stomach.

Anoche fue difícil. La sensación de tener algo atascado en el pecho era insoportable. Finalmente obtuve alivio cuando busqué remedios caseros para el reflujo ácido. Mastiqué chicle de menta y vomité mi cena al instante. No estoy seguro si esto es solo reflujo ácido o algo más serio. Voy a ver a un médico mañana. Hoy intenté beber/comer. Tomé un bocado de sandía pensando que sería ligera, pero fue tan lento de bajar que me asusté y me detuve. Más tarde pensé en probar el helado porque no es sólido y pensé que el frío se sentiría bien en mi garganta. Lo mismo no se sintió bien y me detuve. Con los muchos años que he tenido esto, los alimentos que parecen causarme problemas son cualquier tipo de carne... el pollo y el cerdo son los peores. La pasta y los fideos a veces lo han causado. El pan y las papas fritas. Esos son los principales, ha habido más, pero no he llevado un registro real. Espero que no sea acalasia o algo que requiera cirugía. Eso sería una pesadilla para mí ya que tengo tres hijos pequeños de los que cuidar. Todo lo que puedo hacer es esperar, supongo. ¡Espero obtener algunas respuestas pronto! ¡Gracias de nuevo por su ayuda!

I find that foods which become gummy / paste like, are the worst and get stuck often.  So breads, mushy chicken potatoes, stuffing.  Also some of us do much worse with hot or with cold.  For me cold locks me up, and warm relaxes and gets things to go down easier.  You might try a nice warm soup with small bits in it, and see how that goes.  At least to keep up your energy until you get this figured out.  Also, avoid soda as the fizz can build up in there.  Let us know what you find out.  And don't stress too much, I had 3 small kids too at the time and did just fine after I got treated.

Bebo Coca-Cola todos los días y en todas las comidas. Al menos 6 o más latas al día desde que era niño. Muy adicto a ello. Quizás parte del problema. Creo que lo caliente es peor para mí. Si como carne fría sobrante no es tan malo. ¡Muchas gracias! Es agradable hablar con personas que lo entienden. Mi esposo piensa que es extraño y se asquea cuando hablo de eso y no cree que sea tan serio.

At first I thought I had lost my marbles.  Like, how can I suddenly be bad at eating ?  Take teeeeeny bits with lots to drink, but still it gurgles and makes horrible noise, hurts like a heart-attack and comes back up.  But with treatment and finding out what things are good/bad for you it gets much easier.  I can even go eat in a nice restaurant and not have to sit near the bathroom LOL.  Hang in there.  The most important thing is to get a Gastroenterologist who has lots of experience with Achalasia.  Treatment outcomes are much better after a surgeon has done 50 or more procedures.  Getting it just right is something they are better at with experience.  They will want to do 3 main test for A.  The barium swallow where they can see in real-time if things get stuck.  A scope where you are sedated and they look down there w a camera, and the gold standard test is the manometry which prints out a graph of what your muscles do when you swallow..  That is the definitive test to know if it is A for sure.  Often you can do all 3 in one day and know the results right then.  Best of luck and let us know how you do.

Recuerdo cuando era niño, mi papá usaba Coca-Cola para eliminar los residuos de ácido de la batería en sí. Ahora, piénsalo. Si la Coca-Cola puede hacer esto al ácido, ¿qué le habrá hecho a tu estómago y al esófago? Realmente no creo que hablar de tus problemas esté ayudando mucho. Solo necesitas ver a un MD y él te guiará.

Fui a la sala de emergencias porque no podía conseguir una cita con un médico para una derivación en 3 semanas... no quería esperar. No pudieron hacerme ninguna prueba. Él dijo que parece acalasia sin que yo siquiera le dijera que eso era lo que pensaba. Me puso en una dieta líquida. Tengo una cita mañana con un especialista en un hospital de primer nivel en mi área con cirujanos que se especializan en acalasia.

That is awesome news !!  This thing isn't much fun to deal with but I promise you, once you get treated and learn about what works and doesn't work for you, it really is much better.  I barely think about it anymore unless I have a bad day and need to be more careful with my food choices.  I wish you all the best.  Let us know what they find out.  Will keep you in my prayers.

¡ESO ES GENIAL! Me alegra que hayas seguido mi consejo y hayas ido a la sala de emergencias, lilblondie. Una vez que descarten otras posibilidades, estarás bien. Aprender a vivir con la acalasia no es difícil. No comer ni beber nada, dos o posiblemente tres horas antes de acostarte, es mi regla, y si te atragantas o toses durante la noche, cómprate una almohada en cuña. La cuña te sostendrá lo suficiente para que no estés acostado completamente plano cuando duermas. Tengo que usar una almohada normal encima de la cuña para sentirme cómodo y poder dormir toda la noche. Vas por buen camino, ¡Mucha suerte!
