i am confused

what is going on guys so i have been complaining of this for over a year now, my symptoms are when i have pee feeling i feel it as a pressure in the prostate not in the bladder and when i go to bathroom to pee i am having a hard time "STARTING" a stream just the start but once i go all are normal, flow isnt weak at all but once i finish i have to stay for couple of mins to push even small a mount and what really bugs me the most is that i have to wake up in the middle of the night to pee, feeling big pressure in the prostate but the amount that i pee doesnt really match the feeling its not that much i know a normal person will not wake up for that, went to Urologist about three month ago who asked me to do tests, sperm, blood, ultrasound scan,urine test, urine flow test, all showed normal, the only thing that wasnt normal was the urine flow test, it showed a delay on starting stream, not that big of delay but still theres a delay, prostate size is 16, so he gave me prostatrol i think it called alfuzosin, one pill before bed, first week didnt notice big difference, starting second week it was a downhill run, my symptoms were bad so i had to stop, went back to see him and then he gave me rowatinex , now im on day 3 with it and still same everything, has anyone felt like this before ? btw im 44 fit guy

A 16 gram prostate is normal or actually slightly small. That size prostate doesn’t usually cause urinary symptoms. You may have prostatitus (inflammed prostate) but the urine test would have shown bacteria. There can also be a non bacterial prostatitus, although not common. You may have an enlarged median lobe, blocking flow. That can be seen with a cystoscope. You could also have a stricture in the urethra, which can also be seen with a cystoscope.
The drugs are the correct thing to try, at this point. The drug most people use to improve flow is Tamsulosin (Flomax). I would avoid drugs like Avodart and Finisteride, as they can cause permanent reduced libido, which I assume you wouldn’t want at the young age of 44.
If the drugs don’t help, a cystoscopy may be the next thing to try, looking for enlarged obstructing median lobe, or strictures in the urethra. A multiparametric 3T MRI could also be useful in diagnosing enlarged median lobe, and prostate size, and possible cancer nodules, but it is expensive.
You didn’t mention a PSA test above, so you should have that if you haven’t already. The PSA can help diagnose any possible cancer. If none of the prostate tests, and treatments help, you could look into problems with your bladder.
