Light Adjustable Lens (Post Vitrectomy, Post LASIK)

I am going to soon be having cataract surgery and have a complex situation. My specific question is about the light adjustable lens. I know that after it is implanted, it can be adjusted 3 to 5 times and typically people complete adjustments within 4 to 5 weeks. (Sorry for the length of this message but this is a complex situation). However, if you are willing to wear the UV glasses can you extend the adjustment time? For example, could wait to lock in for 2 or 3 months? This is my situation. In the late 1990s, I had LASIK done. This was extremely successful and I saw 20/20 for years. My left eye was always a little more blurry but overall vision was great. Now, I am in my 60s and I had developed mild cataracts in both eyes. They mostly created halos and starbursts. Of course, I have been using reading glasses for years. That was OKish but about 5 years ago I had to start using computer glasses in order to use my computer. I do a lot of stuff on computer and work on the computer several hours most days. So I went from using reading glasses while reading to wearing glasses almost the entire day. I still had good distance vision. I had my last regular eye exam 2 years ago. My vision was good in the right eye and about 20/40 uncorrected in the left eye. Earlier this year I was starting to think about what kind of lens I would get when I ultimately had my cataracts replaced. My main goal is to not have to wear glasses while using the computer and, preferably, not for distance either. I am willing to have to use them for long reading sessions. I knew that due to my LASIK a multifocal lens like the PanOptix was not ideal. Anyway, several months ago I noticed my left eye vision was getting worse. I actually thought it was due to the cataract. Then, I noticed that I had distorted vision in that eyes including seeing wavy lines. I immediately went to see a retinal surgeon. It turned out that I had a macular pucker in the eye due to a retinal tear. The tear was repaired by laser in May but I was told I would need to have a vitrectomy in order to peel the macular pucker to try to restore some of my vision. At that time, in addition to the distortion, I was seeing 20/60 in the left eye. Before having the vitrectomy I consulted with the office where I did my LASIK surgery in late 1990s. The surgeon also does cataract surgery. After examining everything, they recommended the light adjustable lens for the eye with the macular pucker. The plan would be to optimize it for intermediate computer distance. The right eye would likely get the Vivity lens optimized for distance and then intermediate distance. The success of this really depends on how much improvement I would get from the vitrectomy once the peel was done. If I got back most of my lost visual acuity then this would likely work. On the other hand, sometimes people have the pucker peeled and don't improve. In that case, my right eye wouldn't get much help from the left so this might not work. 2 months ago I had my vitretectomy. The peel went perfectly and the scan of my retina shows that it looks great. Most people get most of their improvement in the first 6 months after surgery. During surgery, they found another small retinal tear (this one had no effect on vision) that was laser repaired during surgery. As a result I needed to have a long acting gas bubble in my eye. The surgery and the gas bubble typically result in rapid worsening of the cataract. I had hoped to weight to do cataract surgery until 6 months was up and we knew how much improvement I would get from the peel. At 4 1/2 weeks after the vitrectomy my vision in my left eye had improved to 20/40 which was better than it had been before surgery. I was ecstatic as that was very quick improvement. Last week though my vision in that eye cratered. At my most recent appoint (7 1/2 weeks after surgery) I was only seeing 20/80 in my left eye. And, my reading and computer glasses now make my vision worse in that eye. I see better with no glasses on at all. I had a new eye scan and my retina is still fine. Basically this is all due to the cataract. My retina surgeon says that as soon as the gas bubble is gone (probably a couple of weeks) it is safe to have cataract surgery. On the one hand, I want it done ASAP since my vision is already so poor in that eye and the surgeon tells me it will only get worse. Also, I will not be able to see any improvement I get from my peel as the improvement is masked by the worsening cataract. From that standpoint I would like to have cataract surgery as soon as possible. But, if, say I had cataract surgery and put in the LAL and then locked it in 4 weeks later I may be locking it in when my visual acuity is still continuing to improve for another several months. Ideally I would like to know what improvement I will get in the first 6 months before I lock in the LAL. At the same time, though, I am not sure I can wait that long to get the cataract replaced. I went from being 20/40 or so to being 20/80 in about a week. So I wondered if I could do the cataract surgery, get the LAL and adjust it a time or two but wait to do final adjustment until closer to the end of the 6 months. I know I would have to wear the UV glasses all the time but that is OK. Also I was told that my surgeon had just started using the LAL with the internal shield to help protect from the UV light. Does anyone know if waiting 2 to 3 months to lock in is possible or if it has to be locked in more quickly? Oh -- the original plan was to do the left eye and then wait about 3 months or so and then do the Vivity lens for the right eye assuming everything went fine and I got visual improvement from the peel of the macular pucker (that seems likely at this point).
