I would be very grateful for any advice on what direction I should take next and which specialists I should see.
My symptoms began in 2015 with joint pain. My legs became swollen, and I developed a lump in one leg that required surgery.
After that, I started experiencing persistent headaches. Over time, I developed severe insomnia, followed by cognitive slowing, memory problems, and widespread numbness in my body.
At one point, the numbness became very severe — I had almost complete loss of sensation across my body. When touching my skin, I could barely feel it. My perception of temperature and normal bodily sensations was also significantly reduced, and my body felt detached and numb.
Later, I developed symptoms of derealization. I feel disconnected from my surroundings and from my own body. My vision also became blurred. I continue to suffer from severe insomnia, persistent low mood, pessimism, and depression.
I also experience marked fatigue, weakness, and very low energy levels. Even simple daily activities can feel exhausting.
I have ongoing severe insomnia and often cannot sleep without sleeping medication.
I experience strong cravings for sweet foods. I have been tested for diabetes and my blood glucose levels were normal.
I was previously assessed by a neurologist. An MRI brain scan showed areas of gliosis; however, I was later told these findings were likely congenital and not responsible for my symptoms.
My main ongoing symptoms are poor concentration, severe memory impairment, cognitive slowing, and confusion. I struggle to process information and function normally in daily life.
During a trip to India for medical treatment, I was found to have low vitamin D levels and elevated vitamin B12 levels.
I have noticed that antidepressants slightly improve my condition — they give me more energy and somewhat improve my memory and thinking. However, when I stop taking them, my condition becomes significantly worse, with severe mental slowing, memory problems, and increased insomnia.
At times, my cognitive state becomes so poor that I do not recognise familiar people. I also experience severe joint discomfort, sometimes described as sharp or “electric shock”-like sensations.
The initial joint pain eventually resolved but was followed by worsening depression. I also developed gastrointestinal issues, including constipation and stomach discomfort.
Over the years, I have tried several antidepressants. Many were ineffective or lost effectiveness over time. Clomipramine initially helped, but was not effective when tried again later.
My symptoms have persisted for many years and have significantly affected my quality of life and ability to function.
I feel exhausted from seeing many doctors without receiving a clear diagnosis. I have not yet been given a clear explanation for my condition.
In addition, I have noticed a gradual reduction in hearing over time.
Based on this history, I would greatly appreciate advice on which specialists I should see next. Should I continue neurological investigations, or consider other possible causes such as endocrine, autoimmune, sleep-related, or metabolic conditions?
