Is this something that’s happened only recently or a problem over a long time? The picture shows that your frenulum is shorter than normal (frenulum breve) which may be the root of the problem. It also shows a large patch of balanitis beneath the foreskin. Were you are aware of this?
If you’ve exhausted simple remedies such as lubrication, rest, etc then I suggest you ask your GP/family doctor to recommend a urologist to discuss surgical options. This might be frenuloplasty or a full circumcision, so best to seek specialist advice.

This has been happening for the past year. I’ve been sexually for 8 years without this issue.
The picture above was taken not long after sex so it was irritated and torn. Here are images 2 days later when it’s healed:


Is there still signs of balanitis? There’s no pain or discharge. I’ve tried searching balanitis but still unsure as google images tend to show the worst case scenarios.
It seems to have settled somewhat, but still redder than surrounding skin.
