What dose my ECG mean?

For the last five years I have been on high blood pressure medication. It has always been a problem for me as I am obese, and as I forgot to mention in my last few questions I also have diabetes. I made the stupid mistake of quitting both my diabetes meds and my high blood pressure meds. I suppose this was part of a new ‘natural medicine’ craze. Yes I know I am stupid for doing that, however, I am on them again as not taking them really didn't help. 

The problem is that for the last few days I feel like I felt like I was getting a cold but it never eventuated, my chest felt tight and painful, I had a strange pain in my jaw and my left hand was achy and tingly, not to mention my breathing is really laboured. 

So I went to the doctor and I had an ECG taken. My doctor was on call so she had to leave half way through, so the nurse gave me the reading when it was done. I took a picture on my phone and cropped it a bit so you can read it. 

The nurse said she didn't really know how to read them so she just gave it to me and made me an appointment for tomorrow. I am feeling worse now and really want to know what it means.

What was the date at the top??

It says 19/08/2004 but it should say 26/05/2014. The nurse said that the machien has been playing up latly as the small hospital were my local doctor is just updated there system. (When I say small, I mean it is pretty much a large house. But I cant really complain) She did a re run so it had the right date for the doctors report but it was acatly the same and so thats why she just gave it to me. I mean, you cant just have the wrong date on offical documents. 

Sorry I forgot all about that.

Dunno what it means but it's certainly showing abnormalities

At my local hospital you nowadays see a cardiologist right after the ECG.

Not sure that it speedily gets the next tests done. Three years ago after one, I then waited seventeen weeks for a stress ecjocardiogram then ten weeks for an angiogram then seven months to get my AVR.

Last year when going into AF and having ECG first week in December

it was the third week of march before getting a cardioversion.

With it now being nurse led it was supposed to just be four week wait.
