James, the small amount of skin that I can see near the top of the shaft of your penis suggests you have severely dry skin. In statistical terms, the number one enemy for healthy penile skin is the vast majority of soaps and other forms of body wash such as shower gel, simply because, most males use these to clean their penis with them. Unfortunately, the skin on the head of the penis is perhaps the most sensitive on the human body - so eventually, years of exposure to things like soap, shower gel, hair shampoo and things like that can have a severe impact on the skin.
Then there is exposure to other chemicals for many males, this can be from ordinary skin cream and lotions, moisturisers of any kind, lubes of any kind, baby oil, regular use of condoms - and so on. When penile skin becomes damaged basically anything that has a chemical base to it will place further demand on an already compromised skin. This can result is overly-dry skin, cracked skin and what many refer to as balanitis - specifically redness and irritated looking skin due to dermatologically derived problems.

The best approach is not to use ANY SOAP on the Glans / Head of your penis - wash with water only EVERY time you have urinated, after sexual arousal, and ejaculation. If you are not circumcised, protect the Glans of your penis every time you take a shower by drawing the foreskin as far forward as you can, then wash everywhere else, thoroughly rinse off all the gels / shampoo and suchlike, then expose your glans wash it with the water only using the inside of your hand, then each time you have washed it you must make sure to 100% pat dry immediately afterwards with a clean towel that only you use.

After you have put this in place and you still have the situation going on you need to look at what else your penis is coming into contact with on a fairly regular basis.  The likely suspects are:-

  • any chemical compounds such as skin cream / lotions, baby oil, oil based lubes, chemicals inside condoms (e.g. easy-on and spermicides)
