Unfortunately the photo is too poor to tell what is going on. If you can capture a photo in clear daylight that would help (next to a window during the day, for example).
So, it could be irritation or an actual infection if this is right on the Meatus (pee-hole) itself. Because of the location you should not try to apply any creams or lotions to this as this can potentially cause an issue for the urethra (tube inside the penis).
The thing I would suggest is making sure you fully clean up after each time you urinate. When you are at home use a clean dry towel that only you use after washing with water only. When you are out of the house and you urinate either carry paper tissue or go into a toilet cubicle and get a sheet or 2 of toilet paper and make sure you remove any traces of urine from the head area of the penis.
If you are not circumcised make sure the foreskin is well out of the way before you start to urinate.
Another problem time can be if you are sexually active or if you masturbate regularly at the moment. Not only is there a great deal of friction going on for the skin in the area, but pre-ejaculate and semen, obviously, can really irritate what you have a great deal. Condoms would certainly irritate this further - if you can abstain from sex and masturbation for a week or 2, or until this has cleared away.
Should it get worse then get a Doctor to check it for you.
