Skin issue on the penis - what should i do?

* White rough area under the foreskin, almost scaly, suggest lots of tiny bumps clustered together. In the crease where the head meets the shaft. Discovered 2 weeks ago, not itchy or uncomfortable. * Last sexual activity was protected and over 6 months ago. Tested 4 months ago (negative for HIV, Syphillis, Chlamydia, Gonorrhoea). * I'd say i have good genital hygene, and i clean under the foreskin. * A mouth specialist found lichen planus in my mouth late last year if this is useful to know? * The GP i spoke to referred these images to a Dermatologist who has suggested it could be lichen planus or a wart and has prescribed a strong steroid cream. But after a week of using it there is no change, in fact it may be worse. Can you help please?

Hi Robert,

This looks like a genital wart, but as a rule warts DO NOT BLEED!
So a bleeding warty lesion needs to be referred and seen by a dermatologist for a biopsy under the TWO WEEK RULE protocol. If you live in the UK, please mention to your GP that this warty lesion bleeds and that you need to be referred under the TWR protocol, irrespective of COVID-19.

I can reassure you it is not lichen planus, if necessary re-send the second photo to your GP as it’s clearer than the first.

The bleeding is not as a result from the skin condition. In fact i regulary get a little redness/bleeding from friction in this area over the years. In this case it is sandwiched between the two white areas so it may be under more friction than usual.

Nothing has come out from the wart itself yet.

The appearance of this warty lump is somewhat atypical, which is why I’m anxious for you to see a specialist. You should mention that it bleeds from time to time, as this is a ‘red flag’ for a more thorough assessment of the lesion- i.e. a biopsy.
