Red Spots on Penis from last 3 months which come an go, what should I do

Hello, Somewhere in Feb , I saw a red rash under my foreskin and consulted a urologist who mentioned it was Candida infection and gave me Fluconazole 150 mg and Ketoconazole (2% w/w) cream to apply on affected area for 5 days daily. Applied with no relief the rash will not go. Consulted another Urologist and he suggested to have fluconazole 150 mg alternate days for 7 days and apply Luliconazole (1% w/w) instead of Ketoconazole cream. Worked for me but as soon as I completed my 7 days, the rash again started to re-appear but this time on my Penis glans. Visited the doctor again and this time he mentioned only to apply Luliconazole cream and not to take oral medication anymore. I cleaned the area with Dettol soap and the red spot reappeared. This time consulted Dermatologist, he mentioned that it is Candida Balanitis and I should take Fluconazole 400 mg once a week and keep applying Luliconazole (1% w/w) daily for 3 weeks. Clean daily and pat it dry. Completed 3 weeks on 27th May. Today, I see that the spots have again started reappear. These spots become very light when I have a erection but are more prominent otherwise. I have been cleaning my penis only with warm water ever since I made a visit to Derm. Please help to figure out what is this. Attaching pics that I took today. BTW, Derm had mentioned to get Herpes test which came out negative.

In similar position, only my rash started and stayed on the glans. Rash appear and dissappear within minutes to couple of hours. Never lasts more than a few hours on a certain spot on the glans. Tested positive for HSV1, but according to the Dr, not genital hsv1 as per the bloodtest conducted. Urine culture clean.
Apply Protopic 0.1% as per Dermatologist prescription at night time. Rash tend to stay away, but if I slip an application it appears again.
So far the Protopic seems to be the only sort of protection as its not the cure.
Funny enough that I am HSV1 positive, you negative, so seems Herpes is not the culprit.
How did your rash start, unprotected sex?? Mine started 4 weeks after unprotected oral. Not very sexually active, first oral in 18 months.

No unprotected sex. It seems I passed out at night due to wet dreams and realized this in morning. Got up and cleaned it but it seems by that time, rash was already there on by inner foreskin. It has been since then. Started on foreskin and now has moved onto glans. Similar as yours, the moment I apply Luliconazole, fades away in few hours time and when I stop or miss application, appears next day
